Note: The maximum amount of Supplemental Life insurance that may be elected is $500,000. Employees electing any amount above $200,000 must complete and mail the Prudential Evidence of Insurability (EOI) form by October 16. Employees currently enrolled in Supplemental Life may increase their coverage by $10,000 without providing the EOI form.
